• All students with documented life threatening allergies, asthma, seizures or diabetes will be provided with a care plan
• Other conditions will be care planned at the parent/nurse discretion
• Care plans are updated annually

Severe Allergies

• Epinephrine in a labeled box from pharmacy with medication consent form
• Benadryl with a medication consent form
• Allergy action plan from medical provider

Asthma

• Inhaler in the labeled box from the pharmacy with medication consent form
• Asthma action plan from medical provider

Seizures/Diabetes

• Diabetic Medical Management Plan from medical provider
  • Diabetes supply checklist (coming soon)
• Seizure Action Plan from medical provider
  • Rescue medication with a medication consent form is needed

G Tubes

• Medication and/or feeding orders from the medical provider

Wayland Union Schools is once again partnering with Wayland Village Pharmacy to offer staff and students the opportunity to receive an optional annual influenza (flu) vaccine during the school day. This year’s vaccine is designed to protect against all three influenza strains expected to circulate during the upcoming flu season.

Cost & Insurance

There is no out-of-pocket cost for individuals who are able to bill the vaccine through their insurance.

For those not using insurance, the cost is $35 per vaccine. Payment can be made by cash or check. Credit card payments are also available by contacting Wayland Village Pharmacy in advance.

Student Consent Forms

For your child to receive the influenza vaccine at school, a completed consent form must be submitted to your child’s school main office by October 8th, 2026.

If you do NOT wish for your child to receive the vaccine, no action is required.

Clinic Date

The 2026 Influenza Vaccine Clinic will be held during the school day on Wednesday, October 14th, 2026.

Medication Administration Form

• Prescription medications require a provider's signature
• ALL medication forms require a parent/guardian signature
• Only one medication for one student per signed medication administration form

Self Carry Authorization Form - Rescue Medication

• Self carry must be authorized by the prescribing physician
• Self carry applies to emergency rescue medication (epi-pens, inhalers, diazepam, etc.)

October 2026

• All medications must be in the original container
• Medication must not be expired
• The pharmacy label must be intact for prescription medications
• OTC medications must be unopened containers
• Only a 30 day supply of prescription medication may be accepted

Fever

• Students should stay home if they've had a fever of 101F or higher in the past 24 hours
• Students may return to school when they have been fever free without the use of medication for 24 hours

Vomiting/Diarrhea

• Students should stay home if they have vomited 2+ times OR has had 2+ loose stools in the past 24 hours
• Students may return when vomiting/diarrhea has cleared for 24 hours and they are able to eat/drink again

New Cough

• Students should stay home if they have a new persistent cough that interferes with their ability to participate in school activities, has difficulty breathing or trouble catching their breath
• Students may return once they are feeling better and showing signs of improvement for 24 hours

Rash

• Students should stay home if the rash is new and undiagnosed by a healthcare provider, is increasing in size drainage cannot be completely covered or accompanied by fever
• Students may return once the rash is healed, or they are cleared by a healthcare provider, or any draining is able to be completely covered

Eye Redness/Draining

• Students should stay home if they have new and unexplained eye redness, drainage, fever, eye pain, eyelid redness or vision changes
• Students may return once symptoms have gone away or they have been cleared by a healthcare provider
